Repetitive friction on the outer knee.
The iliotibial band (ITB) is a long, strong, tendon-like structure that originates from the hip muscles and runs along the outside of the thigh to just below the outer knee. IT Band Friction Syndrome is inflammation and painful irritation where the band crosses the outer knee joint.
It's most common in endurance runners, who bend and straighten the knee thousands of times per race, and can also affect cyclists with poor bike setup, or teens during rapid growth.
Contributing factors often include abnormal hip, knee, or foot mechanics, rapid increases in mileage, poor hip strength or endurance, limited flexibility, worn-out shoes, or training on uneven surfaces.
Conservative treatment is highly effective: a physical therapist assesses your walking, running, bike fit, flexibility, strength, and foot mechanics, then builds a plan combining rest, stretching, strengthening, and sometimes a shoe change.
A full movement assessment, not just rest.
Because ITBS is driven by mechanics as much as mileage, treatment starts with a thorough evaluation of your running or cycling form, hip strength, and flexibility, then builds a targeted correction plan.
